By signing this document, the student, site mentor, and internship coordinator agree to the conditions stipulated about the internship as outlined in the syllabus and this document. If the site mentor or internship coordinator, at any point feels the student is not fulfilling the requirements and expectations that have been outlined, steps will be taken to quickly resolve any issues. If those issues continue to persist, the site mentor or internship coordinator has the right to terminate this agreement, which most likely will result in the student failing the course.For a full description of the internship, course objectives, and expectations, please see the course syllabus. Please Note: Students cannot begin at the internship site until the completed contract has been returned to the internship coordinator.
